• Duly accomplished and notarized visa application form and photocopy
• If not a UK Citizen, applicant must show proof of legal residence in the UK, in addition to the valid Foreign Passport. (2 photocopies)
• Passport valid for at least six (6) months beyond the intended period of stay in the Philippines and a photocopy of its data page
• Copy of hotel booking of the applicant, if there is any (2 photocopies)
• 1 passport size photo with white background taken within six (6) months to the time of application
• A copy of flight reservation or itinerary of travel (2 photocopies)
• Detailed itinerary of activities in the Philippines (2 photocopies)
• Latest bank statement with at least £1,000.00 balance (2 photocopies)
• Self-addressed stamped envelope with tracking number (special delivery)”
